10/5/2023 EnchantaVaughn Approved Upon arrival to the school setting, the client appeared to be in a content state as evidenced by greeting QMHP-C with moderate emotions. The client exhibited difficulty with remaining calm and responding appropriately to authority by not keeping his hands to himself during morning station work. During midday observations, the client exhibited difficulty with staying in his seat and refusing to complete his work. During afternoon observations, the client remained calm and complaint while following directions on the first prompt in his class room. QMHP-C greeted client upon arrival to the school setting. During morning observations, QMHP-C redirected the client. QMHP-C reiterated the importance of remaining calm, responding appropriately to authority, and keeping his hands to himself. During midday observations, QMHP-C redirected the client and reiterated the importance of staying in his seat and completing his classwork. During afternoon observations, QMHP-C provided the client with verbal praise. QMHP-C encouraged him to continue following directions on the first prompt while being calm and complaint. The client responded well towards Therapeutic Day Treatment services throughout the day as evidenced by engagement and positive feedback. During morning observations, the client was unreceptive to redirection by becoming extremely angry and yelling. The client was then able to calm down and agreed to remain calm and respond appropriately to authority by keeping his hands to himself. During midday observations, the client was receptive to redirection. The client agreed to stay in his seat and complete his classwork. During afternoon observations, the client agreed to continue following directions on the first and to stay calm and compliant.
